New OEM Genuine Toyota Door Mirror, mirror less the box...not aftermarket junk!  It was stored in a bin with other mirrors with left paint scratches and minor marks on it - no cracks or chips or damage. 

Note the fitment - this is very important for the mirror mount and the plug will differ otherwise!

Fits:
2004-2006 Tundra Crew Cab models only, Limited model only (the mount is taller on crew cab)
2001-2006 Sequoia SR5 base models only - the plug is different totally otherwise.
